Win a Connecticut public works contract with between $150,000 and $999,999.99 in state funding and you file the Good Faith Efforts Short Form Plan with the Bid Tabulation Worksheet. Cover page

Company identity (name, address, telephone, plan preparer's and head of company's email), the project (name, State Contract/Project No., project owner, Construction Manager if applicable), an Original vs Revised Plan checkbox, and the money: Total Project Contract Value, the State Funded Portion, and the Assigned MBE and SBE Values as both a percent and dollars — the project-specific Spending Allocation Goals from your bid notification.

Section 1 — Project description, timeline, and trades involved

A 1–3 sentence project description; start and end dates each flagged Actual or Estimated; and three one-item-per-line lists covering only what your company self-performs or self-provides: trades-related work, manufactured/self-provided supplies, and non-trades services ("N/A" is acceptable). A checkbox certifies you attached the awarding agency's bid-document excerpts supporting the lists. Subcontracted work belongs in Section 2, not here.

Section 2 — Subcontractor/Supplier/Vendor Availability Analysis

The heart of the form: three separate tables — trades, materials, and services — each with nine columns (A–I):

  1. Type of Trade / Material / Service (be specific: "wire, cable", not "electrical supplies")
  2. Company Legal Name — every firm you solicited
  3. DAS Certification: WBE, MBE, DisBE, SBE, or "non-DAS Certified"
  4. Date Solicitation Sent
  5. Date(s) of Additional Solicitations
  6. Date Bids Due
  7. Date Bid Received
  8. Outcome — "Awarded", or a concrete reason; CHRO calls "Bid Received", "Called/Left Message", and "Said Will Bid" insufficient
  9. Bid Amount

After the tables, two yes/no questions: did you give written notice to a reasonable number of S/M/W/DisBE firms at least 10 days before bids were due (justify any "no"), and did you follow up the initial solicitation? Best practice per the form's own instructions: solicit at least three certified firms per item, search the DAS Supplier Diversity database with simple, varied keywords, and document why any item wasn't solicited. If you didn't award the lowest bidder, attach an explanation.

Section 3 + Attachment III — MBE goals and timetables

Every firm marked "Awarded" in Section 2 lands in Attachment III: Part A (MBE/WBE/DisBE), Part B (SBE), and Part C (non-DAS certified), each with the DAS certification expiration date, the date the contract was executed, and the contract value. You list your own company with its remaining value (total contract minus all awarded subcontracts). Percentages are computed on the state-funded portion, and a current DAS certificate must be attached for every Part A and B firm — expired certifications don't count toward goals. Once approved, changes to Attachment III require a written request to CHRO with documentation.

Section 4 — Documentation checklist

Six checkboxes confirming the attachments: bid-document excerpts for §1 and §2, source documentation for inventory-provided materials, why-not-solicited and why-not-lowest-bidder documentation, and the DAS certificates for §3.

Sections 5–7 — Prompt payment, closeout, and the signed statement

§5 acknowledges the Prompt Payment Policy (subs paid within 15 days past the contractual due date absent a bona fide dispute). §6 acknowledges the five closeout deliverables due within 45 days of substantial completion — see the closeout checklist. §7 is the Concluding Statement: a pledge of best good faith efforts signed and dated by the head of the company with printed name, title, and company name.
