The CHRO Good Faith Efforts Long Form Plan — and what changed in 2026

The 2026 change most guides haven't caught up with
Earlier versions of the Long Form required workforce/EEO sections — an organizational chart, a Company Employment Statistics Report, an availability & utilization analysis, an apprenticeship section. The Long Form effective 07/01/2026 dropped all of them. The current official template matches the Short Form section-for-section — cover page, §1 project description, §2 availability analysis, §3 + Attachment III, §4 documentation checklist, §5 prompt payment, §6 closeout, §7 signed concluding statement — and adds exactly one thing: the Good Faith Efforts Attachment.
Who files it
- Projects with $1,000,000.00 or more in state funding, and
- any Construction Manager on a covered project with more than $150,000 in state funding.

The one addition: the Good Faith Efforts Attachment
Between Sections 2 and 3, the Long Form asks for written answers (verbatim prompts):
- "How did your company identify and select portions of the work to be performed by S/M/W/DisBE companies? Specifically, how did your company break down contracts into economically feasible units to facilitate S/M/W/DisBE participation?"
- "How does this approach increase the likelihood of S/M/W/DisBE participation?"
- "Explain how you provided interested S/M/W/DisBE companies with equal access to specifications, plans, requirements of the contract, addendum and scope clarifications."
- Did your company host a pre-bid meeting or "meet and greet"? (Yes/No)
- Has the plan preparer attended a CHRO Contract Compliance Technical Assistance (TA) training? (Yes/No, with the date)
This is the most judgment-heavy part of either form — the answers are yours, but they should be backed by the same outreach evidence Section 2 documents: bid packages sized so certified firms can bond them, the same plan-room access and addenda for every solicited firm, and follow-up you can point to.
Everything else: the Short Form spine
The remaining sections are identical to the Short Form — read the section-by-section breakdown: the cover page with the assigned M/W/DisBE and SBE values, §1's self-performed lists, §2's three 9-column availability tables with the 10-day-notice and follow-up questions, Attachment III's Parts A/B/C on the state-funded portion (the Long Form also totals A&B&C combined), the §4 documentation checklist, and the §5–§7 acknowledgments and signed concluding statement. Closeout deliverables are due within 45 days of substantial completion — see the closeout checklist.
